Long-Term Attendant Care

Within the Home

Aide care bridges the gap between active rehabilitation and the move toward independence. At Residential Placement and Consultants, Inc. in Ann Arbor, Michigan, we provide in-home aide care 4-24 hours a day.

We believe that staff development plays a vital role in quality client care, which is why all our employees are highly trained with a focus on brain injury rehabilitation. By giving access to education, we help enhance our staff’s understanding of a patient's medical, physical, and cognitive potentials.

How It Works

We evaluate each client and generate a treatment plan by taking into consideration his or her individual needs. Our staff will provide a carry-over of treatment modalities established by the treatment team, which may include physicians, therapists, family, caregivers, and client.

The Life Skills Trainer will assist the client with ADLs, budgeting, grocery shopping, meal planning and/or preparation, compensatory strategy training, and overall monitoring within the home setting.

Internal Case Coordination

Clients with acquired neurological deficits generally have difficulty with organization and suffer from short-term memory loss. As a result, they may require help in organizing and managing their appointments, therapies, and planned events. This is where we come in.

We understand that internal case coordination is essential for consistency and continuity of care and helps ensure optimal opportunities for success. In order to decrease staff dependency and maximize independence and self-sufficiency, the case coordinator works with the patient using compensatory strategies. He or she also facilitates communication between the insurance company, client, doctor, outpatient treating facility, and family.

Additionally, Residential Placement and Consultants, Inc. facilitates weekly team meetings that are designed to promote consistent communication and coordination of care.
